Forms: Effortless Data Collection
Skizaa Forms allows organizations to easily collect and analyze data with customizable forms that include both standard and logic-driven fields. Users can access the forms from both mobile and desktop devices. After collecting data, the information is automatically organized into a table format, which users can then analyze using Skizaa AI and save insights to the dashboard.
My Role – UI/UX Designer
As the sole UI/UX Designer for Skizaa Forms, I was responsible for designing the entire form creation, response, and data analysis flow. I worked closely with product managers and engineers to bring this idea to life, ensuring that the interface was user-friendly, intuitive, and aligned with the overall product vision.
Form Creation & Logic Integration
Form Creation allows users to design both simple and advanced forms using an intuitive drag-and-drop interface. Users can apply logic rules such as conditional questions based on previous responses, allowing for highly customized data collection. My goal was to ensure that form creation was simple for beginners, while also offering enough flexibility for more advanced users who need complex logic.

Ensuring Skizaa Forms is accessible across both desktop and mobile devices was a key focus in my design. I worked on responsive design principles to ensure that users could easily access, fill out, and track their forms whether they were on a laptop, tablet, or mobile phone.
